Job summary

Community Relations Specialist position for City Human Services based at Sunnyside Health Center in Houston, TX. Schedule is Monday–Friday, 8:00 am–5:00 pm. Valid driver license and 6 months of experience required; high school diploma or equivalent essential.

Qualifications

• Six months of experience required.
• Basic knowledge of grammar, spelling, punctuation, and fundamental math functions, including percentages and ratios.
• Valid Texas Driver License required.

Responsibilities

• Serve as a liaison between the community and City Human Services.
• Provide information, referrals, and connections to appropriate community resources and services.
• Support older adults and community members in accessing available programs and services.
• Provide services and outreach to seniors at the Sunnyside Health Center.
• Occasionally visit patients in their homes to check on their well-being and needs.
• Connect seniors with appropriate City Human Services programs and community resources.
• Facilitate coordination and communication between the senior program and Congregate services.
• Participate in community outreach and engagement activities.
• Maintain positive and professional interactions with seniors, patients, families, and community partners.
• Perform other duties as assigned.
