Job summary

Maryland Wellness in Havre de Grace is seeking a Rehabilitation Counselor to organize services that support client recovery and life goal achievement across home, school, workplace and community.

The role emphasizes IRPs, regular client meetings, and collaboration with other providers, with a focus on compassionate, confidential care to advance clients' growth within a supportive team environment.

Qualifications

• Ability to strategically, logically, and realistically analyze, evaluate and develop solutions for client's rehabilitation needs.
• Excellent interpersonal skills with the ability to effectively build relationships with individuals of all ages, ethnicity, education, and work experience.
• Understands and maintains confidentiality and all regulations regarding HIPAA and 42 CFR Part 2.
• Exhibits professionalism and a friendly demeanor, and is a positive representative of the company over the phone and in person.
• Solid written and verbal communication skills.

Responsibilities

• Meet with clients regularly to work on goals (six times a month for adults, four times a month for minors).
• Develop and implement Individual Rehabilitation Plans (IRPs) outlining recovery goals.
• Conduct in home or community visits to provide case management, counseling, coaching, and skills training.
• Link clients to government and community resources and assist in obtaining those resources.
• Ensure client maintenance of physical and mental health and well being.
• Coordinate transportation to and from scheduled appointments.
• Complete all documentation including notes, monthly progress summaries, and quarterly IRP reviews.
• Report regularly to the supervisor and engage in supervision and training.
• Ensure compliance with agency policies and state regulations governing Psychiatric Rehabilitation Services.
• Maintain communication with staff and affiliate organizations regarding scheduling and client care.
• Attend scheduled meetings and trainings on time.
