Under supervision, the Peer Bridger will maintain regular contact, support discharge planning, and facilitate weekly peer groups in hospital or community settings, with travel to multiple locations as needed.

Qualifications

• Must possess advocacy skills and documentation abilities.
• Effective communication and writing skills are required.
• Familiarity with confidentiality laws is preferred.

Responsibilities

• Maintain own wellness and communicate with supervisor.
• Coordinate activities with referred individuals nearing discharge.
• Facilitate weekly peer support groups in hospital or community.
