Job summary

HALI is seeking a Peer Bridger to support individuals in Stony Brook University Hospital’s CPEP and during transition to community care. This role leverages lived experience to provide recovery-oriented support and linkage services under supervision.

The position partners with HALI and Stony Brook staff to deliver peer-led recovery and resource navigation, adhering to program guidelines and focusing on social justice and empowerment for participants.

Qualifications

• Strong understanding of system resources and local Suffolk County resources.
• Trauma-informed peer support, WRAP, Motivational Interviewing.
• Excellent written and verbal communication; comfortable public speaking.

Responsibilities

• Provide support to individuals in Stony Brook CPEP onsite.
• Follow up with assigned members in the community, including home visits and transitions to community care.
• Provide peer support, advocacy education, and link to community resources.
• Provide information and documentation to Stony Brook University Hospital staff.
• Adhere to HALI and hospital regulations and guidelines.
• Engage in co-supervision with HALI and Stony Brook Supervisors.
• Deliver intensive, time-limited peer support and service linkage for enrollees.
• Maintain ongoing engagement to develop recovery plans and connections to supports.
