Job summary

Jobtailor is seeking a dedicated Social Service Coordinator to oversee programs for eligible individuals in the community. You will coordinate group activities, liaise with local agencies, and maintain an up-to-date resource directory to connect participants with essential services.

Qualifications

High School Diploma or GED required.
Bachelor's degree in social work or related field preferred.
Two or more years of experience in social service delivery with elderly, disabilities, or family populations.
Demonstrated working knowledge of supportive services and local resources.
Ability to advocate, organize, problem-solve and deliver results.

Responsibilities

Coordinates programs and activities for eligible individuals on a group basis.
Serves as liaison to community agencies and networks with service providers.
Develops and maintains a Resource Directory of local/state providers.
Organizes educational events on health care, life skills and referrals.
Identifies partners to provide health services and screenings at participants' homes.
Assists participants in building informal support networks.
