Qualifications

• High School Diploma or GED required.

• Two or more years of experience in social service delivery with elderly, disabled, and/or family populations.
• Demonstrated working knowledge of supportive services in the area served by the project.
• Good communication, comprehension and interpersonal skills.
• Computer literacy test passing required.

Responsibilities

• Coordinates social service programs for elderly and disabled populations.
• Develops and maintains a Resource Directory of service providers.
• Organizes educational events on health care, agency support, life skills and referrals.
• Assists participants in building informal support networks with peers, family and friends.
• Performs miscellaneous tasks such as gathering reports or participating in interdisciplinary meetings.
