Job summary

Jobtailor is seeking a community resources coordinator to connect members with community resources supporting their care management journey. You will assist the clinical team and promote health education through outreach and resources.

The role involves non-clinical assessments, referral to care/services, and maintaining resource documentation in line with health plan requirements. Prior experience and a GED are preferred. This is an on-site role in Kansas.

Qualifications

• Requires a High School diploma or GED.
• Requires 1–2 years of related experience.

Responsibilities

• Connect members to community resources to support care management.
• Provide health education and link members to resources.
• Coordinate outreach resources and promote care services.
• Assist clinical team of nurses and social workers.
• Conduct non-clinical health assessments to refer members to care/services.
• Document and maintain community resources per policies.
• Perform outreach to locate hard-to-reach individuals and families.
• Ensure compliance with policies and standards.
