Job summary

Santa Clara Family Health Plan in California seeks a Temp Community Based Programs Specialist to lead and coordinate the network delivering Enhanced Care Management, Community Supports, and Community Health Worker services, addressing social determinants of health.

You will build provider relationships, monitor performance, ensure regulatory compliance, and support data reporting while collaborating with internal teams to improve care delivery for at-risk communities.

Qualifications

• Bachelor's degree in public health, social welfare or related field.
• Minimum two years of relevant work experience.
• Project leadership or management experience.
• Knowledge of home and community-based services and networks.
• Deep understanding of SDOH affecting Santa Clara County.
• Creative and strategic thinker with ability to develop solutions.
• Ability to gather data, analyze, and write reports.
• Ability to interpret rules and set priorities.
• Advanced Microsoft Suite skills.
• Excellent oral and written communication.
• Ability to facilitate meetings and present to leadership/regulators.
• Ability to work under pressure and prioritize tasks.
• Initiative and good judgment in decision-making.
• Compliance with SCFHP policies and procedures.
• Job safety awareness.
• Valid California driver's license.
• Knowledge of community-based delivery system and managed care.
• PMP certification (D)

Responsibilities

• Develop and maintain updated knowledge of community-based services and ECM/CS/CHW provider networks.
• Identify needs to support ECM/CS/CHW delivery and infrastructure.
• Support ECM/CS/CHW delivery per DHCS definitions and reporting.
• Oversee provider network development and relationships.
• Communicate with providers to identify issues and opportunities for improvement.
• Coordinate with internal units on training, payments, and support.
• Schedule and report on regular site visits with providers.
• Provide orientation and ongoing training to providers and staff.
• Track compliance with work scope, objectives, and metrics.
• Generate reports and assist with special projects.
• Troubleshoot provider claims submission and data issues.
• Understand and track regulatory requirements.
• Attend off-site meetings as needed.
• Perform other duties as required.
