Qualifications

• Bachelor’s degree in Social Work, Counseling or Psychology with 3 years’ experience.
• Master’s degree preferred.
• Understanding of pregnancy, birth and parenting issues.
• Knowledge of community resources and family systems.
• Familiarity with contracts and grants funding for PPS services.

Responsibilities

• Provide individual and family case management, counseling and educational services.
• Maintain a caseload up to 25 active clients.
• Conduct comprehensive assessments to develop targeted service plans.
• Support healthy pregnancies, positive birth outcomes and parenting/adoption options.
• Coordinate with external service providers and deliver crisis intervention when needed.
• Maintain client records, data entry and billing documentation.
• Participate in supervision, staff meetings and ongoing training.
• Adhere to HIPAA and Joint Commission standards.
