Job summary

UniversaCare Inc. is seeking a motivated Physical Therapist (PT) to join our Connecticut Shoreline team, covering Old Lyme through Madison.

You will perform comprehensive evaluations, develop individualized plans, and deliver skilled interventions to improve strength, mobility, balance, and independence. You’ll collaborate with physicians, nurses, care managers, and other clinicians, maintain clear documentation, and adapt treatments to patient goals while ensuring compliance with CT licensing

Qualifications

• Current and valid Connecticut Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Current CPR/BLS certification, as required.
• Strong clinical, communication, and documentation skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation for community-based assignments.
• Previous home health, outpatient, geriatric, or community-based experience is a plus.

Responsibilities

• Conduct comprehensive physical therapy evaluations.
• Develop individualized treatment plans based on patient needs and functional goals.
• Provide skilled physical therapy interventions to improve strength, mobility, balance, coordination, and functional independence.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Educate patients and caregivers on exercises, safety, mobility, and home programs.
• Coordinate care with physicians, nurses, care managers, and other healthcare professionals.
• Maintain timely and accurate clinical documentation.
• Communicate patient progress and concerns to the appropriate members of the care team.
• Provide services throughout the assigned Shoreline territory.
• Maintain compliance with Connecticut licensing and applicable healthcare regulations.
