Masters In Home Care, LLC in Old Saybrook, CT is seeking a compassionate Physical Therapist to deliver expert care in patients' homes. You will develop and execute personalized plans of care in collaboration with physicians, focusing on improving function and quality of life for each patient.

Competenze

• CT PT license in good standing.
• Graduate of a PT program accredited by the American Physical Therapy Association.
• Valid driver’s license in good standing and reliable transportation.
• At least three years of clinical experience (ideally one year in home health care).
• Strong organization and communication skills for effective patient care and teamwork.
• Experience with electronic documentation/point-of-care systems is a plus.

Mansioni

• Deliver physical therapy services in patients' homes.
• Develop and implement plans of care in collaboration with physicians.
• Coordinate care with other healthcare professionals and teams.
• Supervise and mentor Physical Therapy Assistants when applicable.
• Utilize a clinical point-of-care system for documentation and reporting (OASIS data, service orders, timesheets).
