Job summary

NorthLakes Community Clinic seeks a licensed Physical Therapist to plan and administer treatment plans that promote overall fitness and health. You will evaluate patients, set goals, and create care plans in collaboration with the care team.

The role offers part-time or full-time options with a comprehensive benefits package, including medical and dental insurance, retirement contributions, PTO, and LMS/HSAs. Candidates must reside in the service area and hold an active Wisconsin license.

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• 1–2 years of experience in a similar role.
• Active Wisconsin Physical Therapy license.
• EMR experience preferred.

Responsibilities

• Consult with and evaluate the patient to determine goals and needs.
• Diagnose and treat movement dysfunction.
• Create plans of care and document patient records in accordance with regulatory, licensing, payor, and accrediting requirements.
• Provide education on therapy exercise techniques and use of adaptive equipment.
• Provide hands-on therapy such as massage, thermotherapy, dry needling as applicable.
• Work collaboratively with other providers including other therapists, surgeons, and the patient’s primary care provider.
• Clean and maintain equipment to ensure clean and safe work environment.
• Other duties as assigned.
