Job summary

NorthLakes Community Clinic seeks a Physical Therapist to plan and administer treatment plans that promote fitness and overall health in our Wisconsin service area. Position can be part-time or full-time.

The role requires a Master’s in Physical Therapy and an active Wisconsin license, with 1–2 years of related experience preferred. You will evaluate patients, diagnose movement dysfunction, and collaborate with clinicians to achieve goals.

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• Active Wisconsin Physical Therapy license; successful passing of the National Physical Therapy Examination.
• 1–2 years of experience in a similar role (preferred).
• EMR experience preferred.

Responsibilities

• Consult with and evaluate the patient to determine goals and needs.
• Diagnose and treat movement dysfunction.
• Create plans of care and document patient records per regulatory and accreditation requirements.
• Provide education on therapy exercises and adaptive equipment such as walkers or wheelchairs.
• Provide hands-on therapy such as massage and related modalities.
• Collaborate with other providers including therapists, surgeons, and the primary care provider.
• Maintain clean and safe work environment with equipment.
