Job summary

NorthLakes Community Clinic is seeking a licensed Physical Therapist to plan and administer treatment plans promoting overall fitness and health. The role supports residents within the service area, with opportunities for part-time to full-time engagement.

Responsibilities include patient evaluation, movement diagnosis, and creation of care plans, along with education and hands-on therapy such as massage and dry needling.

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• 1–2 years of experience in a similar role.
• Active Wisconsin Physical Therapy license.
• EMR experience preferred.
• Successful passing of the National Physical Therapy Examination.

Responsibilities

• Consult with and evaluate the patient to determine goals and needs.
• Diagnose and treat movement dysfunction.
• Create plans of care and document records in accordance with requirements.
• Provide education on therapy exercises and adaptive equipment.
• Provide hands-on therapy (massage, thermotherapy, dry needling) as applicable.
• Collaborate with other providers including therapists, surgeons, and PCP.
• Maintain clean and safe equipment.
• Other duties as assigned.
