Job summary

WVU Medicine is seeking a licensed pharmacist to provide leadership in patient care across assigned areas. You will guide clinicians on best practices, ensure safe and effective drug use, and oversee dispensing and control of medications.

Qualifications

• Must possess pharmacist license in OH or WV as required by the state board
• State and federal background checks as applicable for regulated areas
• Obtain Basic Life Support certification within 60 days of hire

Responsibilities

• Provide leadership and care for patients; guide providers on best practices and safe drug use
• Dispense and control both prescription and non-prescription medicines
• Complete annual clinical competency evaluations
• Review prescriptions to ensure accuracy and determine formulas and ingredients
• Direct pharmacy staff in dispensing and labeling pharmaceuticals
• Answer questions on drug interactions, side effects, dosing, and storage
• Detect and report adverse drug reactions and medication errors
• Assist nursing and other departments with admission, discharge planning and patient education
• Ensure policies related to drug distribution are followed
• Maintain quality assurance, security of controlled substances, and hazardous waste disposal
• Participate in education of students and residents; maintain professional competence
• Engage in administrative, QA, and research activities; supervise technicians
• Resolve third party rejection problems and resubmit claims timely
• Participate in medication use evaluations and QA initiatives
• Enter patient data into records and charging systems
• Instruct staff on state/federal pharmacy laws
• Plan and implement changes to improve productivity

MINIMUM QUALIFICATIONS

• Must possess current Pharmacist license as required by state board where services will be provided: OH: Pharmacist license through the Ohio Board of Pharmacy WV: Pharmacist license through the West Virginia State Board of Pharmacy

• Obtain certification in Basic Life Support within 60 days of hire date.

PREFERRED QUALIFICATIONS

• PharmD from an accredited School of Pharmacy.
• Medication Therapy Management Certification (MTM) through the American Pharmacists Association (APhA)
• Immunization Certification through the American Pharmacists Association (APhA)
• Certified Specialty Pharmacist (CSP) through the National Association of Specialty Pharmacy (NASP)
• Obtain certification in Basic Life Support within 60 days of hire date.

CORE DUTIES AND RESPONSIBILITIES

• Provides leadership and care for patients in assigned areas of responsibility; guides providers and patients on the best practices and established guidelines for care and assures safe and effective use of drugs through clinical involvement.
• Responsible for dispensing and controlling both pre scription and non-prescription medicine.
• Successfully completes annual clinical competency evaluations.
• Reviews prescriptions issued by Physician, or other authorized prescriber to assure accuracy and determine formulas and ingredients needed.
• Directs pharmacy workers engaged in mixing, packaging, and labeling pharmaceuticals.
• Answers questions and provides information to pharmacy customers on drug interactions, side effects, dosage, and storage of pharmaceuticals, if counseling offer is accepted.
• Detects, reports, and helps to minimize or manage adverse drug reactions and medication errors.
• Assists nursing and other disciplines or departments in admission and discharge planning and patient education related to the appropriate use of medication.
• Assumes responsibility for and makes decisions related to the provision of pharmaceutical services.
• Actively engages patients and clients with respect to customer service programs offered by the pharmacy.
• Actively engages patients and clients with respect to program integrations offered by the pharmacy.
• Identifies problems related to aspects of drug use control and offers constructive ideas for resolution.
• Ensures that policies and procedures related to drug distribution are followed.
• Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs.
• Participates in the orientation and education of pharmacy students, pharmacy residents, and/or new department employees.
• Maintains professional competence by attending and presenting at conferences, continuing educational programs, reading current professional literature and publishing clinical or practice-related experience or research.
• Participates in administrative, quality assurance, performance improvement and research activities of the department.
• Supervises technicians in drug preparation and distribution duties.
• Maintains awareness of third party Rx systems and resolves third party rejection problems promptly, and resubmits rejected claims for payment on a timely basis.
• Participates in medication use evaluations, quality assurance and performance improvement initiatives.
• Enters data such as patient name, prescribed medication, and cost, to maintain pharmacy files, charge system, and inventory.
• Instructs pharmacy technicians, interns and other pharmacy personnel on matters pertaining to pharmacy state/federal law.
• Participates in planning and implementing changes in the pharmacy and improves productivity through participation in staff meetings and projects.

PHYSICAL REQUIREMENTS

• Walk and stand for long periods of time.
• Must be able to lift 30 pounds and push 50 pounds.
• Dexterity required to finger, handle, and reach.
