Job summary

Osage Ambulance District is seeking a Community Paramedic focused on patient-centered, non-emergent care in homes and community settings. The role expands traditional paramedic practice with preventive care, education, and coordination with healthcare partners.

The position reports to the Medical Director and emphasizes mobility, communication, and collaboration to improve access to care and reduce unnecessary ED visits.

Qualifications

• High school diploma or equivalent.
• Active Missouri Paramedic license with valid BLS/ACLS/PALS.
• Experience in community paramedicine, EMS, home health, or care coordination.

Responsibilities

• Conduct in-home and community-based patient assessments and follow-up visits.
• Perform health screenings and medication reconciliation.
• Coordinate care with physicians, nurses, and social services.
• Document encounters accurately in electronic records.
• Participate in quality improvement and outreach programs.
