Mercy Health seeks a Community Paramedic to provide primary care and education in the home environment. The role includes assessing, stabilizing, and arranging transport for acutely ill patients, with care guided by standing orders and physician input.

The position emphasizes patient education on disease management and coordination with physicians, hospitals, and care teams, requiring strong clinical judgment and communication.

Qualifications

• Requires EMT-Paramedic certification or equivalent.
• Experience with patient education and home-based care strongly preferred.
• Ability to work under high-stress conditions and make quick clinical judgments.

Responsabilités

• Examine, screen, treat, and coordinate health services for patients.
• Conduct post-hospital follow-up including monitoring medications and vital signs.
• Document patient conditions and reactions to treatments for physician review.
• Educate patients on chronic disease management (diabetes, hypertension, CHF, COPD).
• Administer patient care per protocols and physician orders.
• Schedule and coordinate follow-up appointments with physicians and hospitals.
• Develop and complete reports for the Community Paramedic Program.
