Job summary

Community Care Network seeks two Community Outreach Specialists to provide individualized support for residents in independent housing and the surrounding community. You will help clients build living skills, access services, and maintain connections to supports while supporting mental health stability.

Responsibilities include coordinating with clinicians and case managers, providing transportation for appointments, and helping clients progress toward goals in their plans.

Qualifications

  • High school Diploma or GED required.
  • Bachelor's Degree in Psychology, Counseling, Social Work or similar field preferred.

Community Care Network is looking for two Community Outreach Specialists to join our team!

Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals, and families through responsive, innovative, and collaborative human service

About The Role

The Community Outreach Specialist provides individualized support to individuals residing in an independent housing program and throughout the community. This position focuses on helping individuals build the skills, confidence, and supports needed to live as independently as possible while maintaining their mental health stability and overall well‑being.

The Community Outreach Specialist assists individuals with developing and working toward goals identified in their individualized plans, building independent living skills, accessing community resources, and maintaining connections to necessary services and supports. This position also provides transportation for day-to-day appointments and other community needs as appropriate.

The Community Outreach Specialist works collaboratively with the individual's support team to coordinate services, communicate needs, and ensure support is responsive to each individual's goals and preferences. This is a rewarding opportunity for someone who enjoys building meaningful relationships, supporting independence, and making a positive difference in the lives of others.

Schedule Options
  • Sunday–Wednesday, 8:00 AM–6:00 PM
  • Sunday–Wednesday, 11:00 AM–9:00 PM
Principal Responsibilities
  • Serve as a role model, mentor, advocate, and motivator to recovering individuals in order to help prevent relapse and promote long‑term recovery and mental health stability.
  • Assist in planning and implementing clients' individual plans of care by coordinating Community Case Management services with Clinician/Case Managers/Service Coordinator as appropriate.
  • Respect individuals' choices and encourages advocacy.
  • Provide a professional and individualized support system that helps to supervise, direct, inform, and assess current and prospective needs of individual.
  • Demonstrate knowledge of individual characteristics such as physical health, mental health, psychosocial characteristics in order to address needs in a personalized manner.
Qualifications
  • High school Diploma or GED required; Bachelor's Degree in Psychology, Counseling, Social Work or similar field preferred.
  • Ability to work alternative work schedules, evenings, weekends, and holidays as needed for operations and at the request of management.
  • Flexibility, dependability, and ability to work independently.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
  • *Successful completion of a criminal history background check after hire.
Requirements
  • Must be able to work Sunday through Wednesday from 11AM - 9PM
