Qualifications

• Bachelor’s degree in a health or social services related field, or equivalent experience, preferred.
• Two years of experience in a community-based setting in health care, social service, or community service.
• Experience in collaborative program development, preferred.

Responsibilities

• Coordinates outreach efforts, engages with community stakeholders, develops partnerships, and implements strategies to increase visibility and impact.
• Develops and implements collaborative programs and services by working with community–based organizations and hospital staff.
• Develops outreach strategies to engage with target audiences.
• Collaborates with team members to create outreach materials.
• Conducts outreach activities to promote programs or services.
• Supports target population in connecting to available programs or services.
• Analyzes outreach data to measure effectiveness of campaigns.
• Builds relationships with community partners to expand outreach efforts.
• Represents the organization at events to increase visibility and awareness.
