Job summary

Easterseals PORT Health is seeking an Education and Outreach Specialist to connect community members with providers across eastern North Carolina. This community-based role requires travel to multiple counties and attendance at events outside regular hours.

The position supports health literacy, care access, and education, with a strong emphasis on serving New Hanover and Brunswick counties. You will work as part of the One Community team in the Trillium service area, collaborating with a

Qualifications

• High School diploma or GED required.
• Maintain valid NC driver’s license and auto insurance for personal and agency vehicles utilized for work.
• Preference for experience with the unhomed population and/or community-based experience.

Responsibilities

• Attend community events, including evenings, weekends and holidays.
• Travel to counties across eastern North Carolina for outreach.
• Serve as a bridge between community members and providers through outreach and education.
