Job summary

BJC Independence Center is seeking a Community Outreach Case Worker to support adults with serious mental illness in the St. Louis area. This 40-hour-per-week role involves community-based services, linkages to housing and food resources, and daily client outreach across communities.

The position requires a Master's Degree in Human Services or related field, 2-5 years of experience, and a valid Class D or E license. Local travel and own vehicle are required.

Qualifications

• Master's Degree in Human Services or a related field.
• 2-5 years of relevant experience.
• Licenses: Class D (IL) or Class E (MO).

Responsibilities

• Provides community support services to complex clients to improve recovery outcomes.
• Completes timely documentation of services linked to treatment plans.
• Contacting individuals and referral sources to re-engage after missed appointments.
• Provides mentorship and job shadowing to colleagues and supports clinical teams.
• Completes annual assessments and other required documentation.
