Job summary

Qualifications

Master's Degree in Social Work from a CSWE-accredited program required.
LSW license in Hawaii preferred; experience may substitute licensure.
Minimum two years' experience as a Licensed Social Worker preferred.
Minimum three years' experience as a Case Manager preferred.
Current Hawaii driver's license and daily access to a car required.
CPR certification and biannual maintenance required.
Ability to adhere to OSHA standards and safety precautions.

Responsibilities

Provide case management services to a targeted population and coordinate linkage to health and community services.
Conduct outreach to locate and determine eligibility for individuals experiencing homelessness.
Collaborate with interdisciplinary team to promote health care resource utilization and health promotion.
