Job summary

American Senior Communities is seeking a Community Nurse Liaison (LPN) to evaluate patients and coordinate with hospital teams for admission to skilled nursing facilities. The role requires weekend hours (Saturday and Sunday 8a–5p) and a remote work arrangement within reasonable driving distance to Indianapolis.

The ideal candidate has at least three years of clinical experience in acute or long-term care and holds a current LPN license.

American Senior Communities is now hiring a Community Nurse Liaison (LPN)

Hours: Saturday and Sunday 8a - 5p

Remote position but must live within reasonable driving distance to Indianapolis

The Clinical Nurse Liaison provides patient evaluations, while collaborating with hospital personnel to determine patient’s clinical needs and appropriateness for admission to skilled nursing facility. This position works closely with our communities and healthcare partners. This position will also be checking benefits and payor sources.
