Job summary

Galloway Ridge in Pittsboro, NC, seeks a Community Navigator to provide psychosocial support for residents and families adjusting to the Arbor, coordinate admissions and care plans, and serve as a liaison with health care providers.

The role requires a Bachelor’s in Human Services, strong organizational, communication and leadership skills, and experience with older adults; you’ll participate in MDS activities, care planning, and interdisciplinary team meetings to enhance resident experience.

  • Ensure care plans are current and reflect residents’ status with MDS coordination.
  • Attend huddles and case management meetings and document progress notes.

Galloway Ridge is an upscale continuing care retirement community offering top-quality care in a warm, welcoming environment—and we’re looking for a compassionate, detail-oriented Community Navigator to join the Arbor team. The Arbor Community Navigator provides psychosocial support to residents and families adjusting to the Arbor, is responsible for managing changes in health status and serves as a liaison between the resident and community agencies. Responsibilities include collecting information in order to formulate resident social history, psychosocial assessment, formulation of resident care plan, completing assigned sections of the MDS, participation in interdisciplinary team meetings, maintaining up to date progress notes, communicating with internal and external health care providers, crisis intervention, assist in in transitions of care, and any other duties as assigned by supervisor. Responsibilities include, but are not limited to:

Administrative Responsibilities
  • Coordinate resident admissions & discharges to & from the Arbor by communicating with local hospitals and Galloway Ridge IDT team.
  • Complete SW assessments along with assigned sections of MDS in a timely manner.
  • Act as an integral member of an interdisciplinary team and serve as a resident advocate and liaison. Attends meetings and collaborates with MDS Coordinator or designee to ensure care plan is up to date and reflects resident’s status.
  • Keeps Manager informed and up to date, collaborates with IL Navigator to transition residents from IL to the Arbor.
  • Informs Navigation Manager of any new grievances and completes them as assigned.
  • Schedule and facilitate family/resident care plan meetings with MDS Coordinator on a quarterly basis, bi-annually, or as needed basis, for residents & enters progress note into PCC for both SNF and ALF residents
  • Attends huddles, weekly navigation and case management meetings, other meetings as assigned
Resident Focused Responsibilities
  • Welcomes new admissions, orients residents to the Arbor, completes required admission paperwork, schedules initial care plan meeting, and enters required assessments into PCC within 48hrs of admission in concert with MDS coordinator.
  • Complete required Medicare paperwork, assists residents as needed with, advanced directives, insurance forms, hospice referrals, and talk therapy referrals.
  • Work with Navigation times to ensure that rooms and apartments are move-in ready, follows the arbor move process guidelines, provides emotional support and adjustment counseling to new residents and their families as needed.
  • All other duties assigned include, but not limited to, enhancing the Resident Experience.

Requirements

  • Minimum of a Bachelor’s degree in Human Services field (i.e. Social Work, Psychology, and Sociology) from an accredited school.
  • A strong desire to enrich resident lives by providing resident-centered care
  • Excellent organizational, communication and leadership skills
  • Ability to work independently and as part of a team
  • Experience working with older adults
  • Experience working in a health setting preferred
