Job summary

Cambridgeshire and Peterborough NHS Foundation Trust is seeking a Band 3 Mental Health Support Worker to join the St Ives/St Neots Older People’s Community Mental Health Team. The role involves home and community visits across Huntingdon and surrounding villages, delivering recovery‑focused care under supervision of registered practitioners.

Qualifications

• Experience in supporting people with mental illness in community settings.
• Must hold a full UK driving licence and have use of a vehicle.
• Willingness to travel around the county to cover Huntingdon and surrounding areas.
• Probationary period applies per NHS CPFT policy.

Responsibilities

• Assist in delivering high standard, person-centred care to service users.
• Assist Care Coordinators in devising and carrying out individual care plans.
• Plan and prioritise visits to delegated patients and record on SystmOne.
• Ensure duties meet quality initiatives and are documented per policy and procedure.
• Record interventions accurately and maintain IT proficiency.
• Support service users with personal care as required.
• Take action to manage urgent risks and summon assistance when needed.
• Report problems to Team Manager/Clinical Lead or care coordinator.
• Develop collaborative relationships with nursing/residential homes and other professionals.
• Assist carers in maintaining their contribution to the service user’s care.
