Job summary

Adult & Child Health seeks a Community Support Specialist to help individuals adapt to community living. You’ll conduct assessments, create treatment plans, and implement interventions utilizing evidence-based practices, including CBT/DBT principles and Motivational Interviewing, with a focus on youth and families.

You will coordinate with healthcare and school partners, document progress, participate in staff training, and contribute to program quality improvement while building strong

Qualifications

• Bachelor’s degree in Psychology, Social Work, or related field; or two years related work experience; or 4-year combination of higher education and two years related work experience; AS in Behavioral Health Support also fulfills requirements.
• Qualifying experience must include delivering services to individuals with mental health or substance use disorders and related tasks.
• Experience in coordinating with families, schools, and community resources is required.

Responsibilities

• Provide community-based mental health services including assessment, diagnosis, treatment planning, and intervention.
• Collaborate with clients, families, and stakeholders to develop individualized treatment plans.
• Coordinate care with other service providers and maintain accurate records.
• Participate in staff development and quality improvement activities.
• Develop relationships with parents, teachers, and other professionals involved in client care.
