Job summary

Jewish Family Service Association of Cleveland is actively seeking a full-time Comprehensive Case Manager to deliver community-based services to adults with serious mental illness and to support their path to recovery. The role focuses on individualized supports and coordination of healthcare, behavioral health, and related services.

The position requires a 4-year degree, a valid Ohio driver's license, and a willingness to travel 75-80% locally on the Eastside of Cleveland.

Qualifications

• Bachelor’s degree in Social Sciences or related field.
• Maintains valid Ohio Driver's License and driving record meeting insurance standards.
• 2+ years of relevant community experience preferred.

Responsibilities

• Provide a wide array of community-based services to adults diagnosed with serious and persistent mental illness to support independence and quality of life.
• Assist clients with goal-directed supports and recovery-oriented interventions as outlined in treatment plans.
• Coordinate care across behavioral health, healthcare, and non-healthcare services; monitor service delivery by other professionals.
