Job summary

Hays County MH Center is seeking an AOT Care Coordinator to support individuals in the Assisted Outpatient Treatment Program. The role involves assessing needs, developing recovery plans, and delivering services in community settings with travel and home visits.

The position requires a bachelor's degree in a relevant field, QMHP-CS credentialing, and timely completion of PMAB, First Aid, and CPR. Flexible hours and travel are expected.

Qualifications

• Bachelor's degree from an accredited college/university in psychology, sociology, behavioral or human services.
• Must be credentialed as QMHP-CS.
• Must obtain PMAB, First Aid, and CPR certification within three months of employment and maintain thereafter.
• Live within 30 miles of the clinic and have reliable transportation.

Responsibilities

• Assess individuals and develop person-centered recovery plans with families.
• Provide skills training, supported housing, employment, psychosocial rehab, and linkage/referral.
• Schedule and deliver individual and group services; maintain documentation within 24 hours of service.
