Job summary

Northeast Michigan Community Mental Health Authority in Hillman, MI is seeking a part-time Community Living Support worker to assist individuals in achieving IPOS goals and community participation. You will help with daily activities, coordinate with the interdisciplinary team, and promote independence in community settings.

Responsibilities include supporting health and safety, documenting progress, and fostering access to recreational and volunteer opportunities.

Qualifications

• High School diploma or GED preferred.
• Knowledge of intellectual/developmental disabilities (I/DD) and mental health issues desirable.
• Must be at least 18 years old and have a valid driver's license and acceptable driving record.
• Ability to work a varied schedule including evenings, weekends, and holidays, and occasional overnight trips.
• Ability to communicate effectively with individuals, families, natural supports, and providers.

Responsibilities

• Provide support and services to individuals as identified in the IPOS.
• Adhere to agency policies for medication setup and/or delivery.
• Assist in finding and maintaining volunteer opportunities.
• Work with providers, families, natural supports, and case managers on assessments, treatment planning, and service implementation.
• Preserve health and safety to support independent living.
• Prepare progress notes, reports, and required documentation.
• Demonstrates knowledge of universal precautions, standards of practice, and emergency protocol.
