Job summary

Achieva in Greensburg, PA is seeking a Community Living Supervisor to coordinate daily operation of Community Home(s), help individuals chart and achieve personal goals, and coach staff. Qualifications

• Associate's Degree - 2+ years of experience as a Direct Support Professional, with at least one year of supervisory experience.
• Bachelor's degree, with some previous experience.
• Ability to complete one-person physical transfers
• Valid Pennsylvania driver’s license
• Must meet Act 33/34 clearance requirements
• Must obtain CPR certification within 6 months of the start date.
• Must pass Medical Administration Training (Provided by Achieva)

• Ensures compliance with the Department's Policies and Procedures relative to Community Homes.

• Assign and ensure completion of each person's Annual Assessment and supporting documents.

Position Summary

The Community Living Supervisor role coordinates the daily operation of assigned Community Home(s). Together with each person, their family, and other team members, assist in charting a course that supports each person in realizing their life of personal significance. Responsibilities also include supporting individual outcomes, coaching staff, and serving as the on‑call contact based on a rotating schedule. This position reports to the Community Living Specialist.

Career Development

• Enroll in the Front Line Supervisor (FLS) certification program
• Receive a pay rate increase ($1/hr) and one‑time bonus upon completion

Key Responsibilities

• Provide direct care supports to people with intellectual or developmental disabilities and specialized care in daily living, social, and recreational activities.
• Ensures compliance with the Department's Policies and Procedures relative to Community Homes. Assembles, maintains, and monitors all in‑house and persons records. Assists with and ensures the upkeep and maintenance of the home.
• Implements formal and informal outcome plans to assist the person in achieving their personal outcomes.
• Develops the weekly staff schedule. Adjusts staff schedules due to vacation, absenteeism or illness with prior approval. Ensures all shifts are covered.
• Communicates and resolves program and individual concerns with the Community Homes Supervisor on a regular basis. Participates in general staff meetings.
• Assign and ensure completion of each person's Annual Assessment and supporting documents, and ensure the implementation of the outcomes identified in the Individual Support Plan.
• Distributes individual funds, obtains and submits receipts, and assists with banking as advised.
• Administer medications (upon completion of certification; all necessary training provided).
• Performs other duties as assigned.
