Job summary

Fraser is seeking a passionate and motivated Program Coordinator to join our Community Living team in Saint Paul, MN. In this leadership role, you will help support individuals served while guiding staff to ensure high-quality care and services.

This position is full-time, 40 hours per week, Sunday–Thursday, 9 AM–5 PM, with on-call rotation. The starting pay range is $50,000 to $53,000 per year, depending on qualifications, plus a $1,000 hiring bonus for external new hires.

Qualifications

• 2 years of professional experience working with individuals with mental health needs.
• 1 year of supervisory and program coordination preferred.
• Valid driver's license required.
• 245D qualified in one of the listed ways (HS Diploma with experience; BA/BS with experience; Associate degree with experience).

Responsibilities

• Coordinate intakes and discharges in adherence with policies and procedures.
• Remain abreast of and follow all regulations and best practices in the delivery of services.
• Communicate staffing needs to Human Resources/Staffing Coordinator.
• Submit all billing to Accounting Department in accordance with the schedule.
• Coordinate services with physicians, mental health professionals, and IDT/special education teams, as applicable.
• Carry and respond to all incoming calls on the Community Living emergency cell phone in accordance with procedures.
• Provide job training, direction, and supervision as required by regulations and Fraser policies.
• Develop training materials and coach staff; promote growth and job satisfaction.
