Job summary

Milestones Services LLC in Louisville, KY seeks a Direct Support Professional to provide direct services in staffed residences, personal care and community living supports as part of a client-centered team.

Your responsibilities include assisting clients to meet goals, administering medication with training, ensuring health and safety, coordinating transportation, documenting progress, and supporting development through social activities and community engagement.

Qualifications

• 18+ years or older with a high school diploma or GED; or 21+ with no diploma required.
• Must have a social security number or valid work permit.
• Proper vehicle insurance coverage is required.
• CPR and First Aid certification is preferred (Milestones offers in-house training).

Responsibilities

• Assist clients in meeting goals and outcomes per the client’s service plan.
• Administer medication with training and consent.
• Ensure health, safety and welfare of clients in home, residential and community settings.
• Complete documentation of progress, hours, MARs, logs and notes daily.
• Engage clients in community activities and social interactions; provide transportation when needed.
• Provide respite care for families as per plans.
• Observe and report changes and safety concerns to the team.
