Job summary

You will assist with personal care, transportation, and program-specific duties, including documenting progress and supporting ongoing planning.

Qualifications

• High school diploma or equivalent is required.
• Zero to one year full-time experience in human services is preferred.
• Ability to communicate with verbal and nonverbal individuals.

Responsibilities

• Teach, model, and coach activities of daily living (ADL) and monitor performance with feedback.
• Plan and facilitate community-based activities and access to resources.
• Provide transportation and assist with community integration.
• Contribute to a safe, therapeutic environment by de-escalating and using behavior management as authorized.
• Collaborate with community systems and other providers to support individuals served.
• Maintain confidentiality and document services timely.
• Assist with discharge planning and staff training as needed.
