Southeastern Directions for Life seeks a full-time Community Life Facilitator at our Education & Integration school site in Sioux Falls, SD. You will guide adults with developmental disabilities in self-care, social, vocational, independent living, and communication skills.

Responsibilities include family communication, advocacy, team meetings, and ongoing documentation. Must be flexible for emergencies and holidays; Southeast operates 24/7.

Competenze

• High school diploma or equivalent; Bachelor's degree in human services or education preferred.
• Must have valid driver's license, car insurance, and acceptable driving record.
• Background checks: fingerprinting, CDSS Central Registry, MVR, and drug screening.

Southeastern Directions for Life has an immediate opening for a full-time Community Life Facilitator at our Education & Integration school location. Southeast's mission is 'empowering people to discover their directions for life in a changing world'.

Job Summary

This position provides services to adults with Developmental Disabilities. You will work directly with the people we support teaching them self-care, social, vocational, independent living, and communication skills. Communicate with family members of the people we support in an effort to maintain a positive relationship between them and other family members. May act as an advocate for the people we support in communicating their needs, self-expression, and goals. Provide community interaction that is safe, clean, and conducive to learning. The CLF is responsible for attending team meetings, program writing, assessments, data, monthly reports, I-Pops, and PCT tools. Southeast operates 24/7, 365 days a year. Flexibility during emergencies and inclement weather is required. Must be able to work holidays when necessary.

Qualifications

High school diploma or equivalent; Bachelor's degree in either human services or education preferred. Must have a current driver's license, car insurance, and a driving record acceptable to Southeast’s insurance provider. Must pass a comprehensive criminal background check to include fingerprinting, Dept. of Social Services Central Registry check, Motor Vehicle background check, and drug screening.
