Job summary

AbleLight seeks a Community Life Coach in Grand Junction, CO to help people with intellectual disabilities connect to their communities through events and volunteer opportunities. You will assess interests, implement service plans, and ensure safety during community activities.

The role requires a high school diploma or GED, valid driver’s license, ability to lift 50 pounds, and completion of the company training. Training and background checks are required.

Qualifications

• High school diploma or GED required.
• Must be able to use Microsoft Office and electronic charting programs.
• Valid driver’s license and good driving record required.
• Completion of Community Life Coach training upon hire.
• Clear background check as per regulations.
• Ability to lift at least 50 pounds and stand for 8 hours daily.

Responsibilities

• Identify community resources and activities matching participants’ interests and skills.
• Facilitate participation in local events and organizations in the participant’s neighborhood.
• Foster connections between participants and non-disabled individuals to increase social capital.
• Accompany participants to activities and support involvement.
• Communicate progress to guardians or care providers; ensure participant safety.
• Provide attendant care or other supports as needed (e.g., medication assistance).
• Support AbleLight’s mission and values; empower participants to direct their own life choices.

Under the supervision of the Manager, The Community Life Coach is responsible for helping people with intellectual disabilities connect to their communities through such activities as community events / activities and volunteer opportunities. The CLC assesses skills and interests for participants, implements service plans, and facilitates the involvement within the community. The CLC assures safety in the community while securing activities that enrich the participant’s life.

Requirements

• High school diploma or G.E.D. required. Must be 18 years of age, or older.
• Must be knowledgeable in Microsoft office and have the ability to use electronic charting programs.
• Must have a valid drivers’ license and a good driving record as defined by AbleLight policy.
• Upon hire, the CLC must successfully complete the Community Life Coach training.
• Clear background check as defined by regulations and policies.
• Ability to lift at least 50 pounds. Ability to do daily standing, bending and lifting.
• Must be able to stand for 8 hours at a time..
• Ability to establish and maintain professional and cooperative relations with local businesses and agencies.

Responsibilities

• Identify community resources and activities pertaining to the interest and skill level of participants on their caseload.
• Facilitate the participation of participants in typical and integrated activities (those used by the general public), events and organizations in the participant’s neighborhood or local community in ways similar to others persons of the same age an interest.
• Foster connections between participants and persons without disabilities to increase social capital.
• Accompany participants to activities, if needed, and facilitate involvement.
• Communicate progress, successes, and challenges to guardians / care providers as needed.
• Ensure personal safety of participants on caseload.
• Provide support to maintain or enhance personal independence.
• Provide attendant care and/ or other supports as necessary (i.e. personal care, medication assistance, etc.)
• Supports AbleLight’s Mission, vision and Core Values while empowering each person to direct his or her own life choices.
• Perform other duties as assigned.
