Job summary

HeartWell Services LLC in Albuquerque, NM is seeking Life Skills Coaches to assist individuals with developmental and/or intellectual disabilities in accessing the community. You will support daily living skills and act as a role model for community integration, sometimes using your own vehicle to travel to activities.

Typical days run Monday-Friday with additional weekend hours; benefits include health, dental, vision, paid time off, mileage reimbursement, attendance bonuses, and training

Qualifications

• High school diploma or equivalent.
• Current and valid drivers license.
• Safe, insured, reliable vehicle.
• Must be 18 yrs or older.

Responsibilities

• Provide transportation using own vehicle for activities.
• Act as a role model for community integration.
• Provide assistance as needed with bathroom care.
• Document and report daily to your supervisor.
• Attend trainings as scheduled.
• Provide emotional support.
• Identify community activities to attend (volunteering, economical events, educational events, etc.).
• Follow an activity calendar.
• Other duties as assigned.

About this position

Description: Life Skills Coaches that are community-based assist individuals with developmental and/or intellectual disabilities in accessing the community. The day is spent supporting an individual in all the areas of daily living as needed or directed by their plans. You have the opportunity to enrich and support another individual's life simply by being available and supportive. You will use your own vehicle to take individuals to a variety of activities throughout the week and work alongside them, teaching important life skills.

We have different opportunities available for our Life Skills Coaches that are community based. A typical day starts at 9am and ends at 3pm and runs Monday-Friday.

• 12 pm - 5pm Saturday supporting a minor with a lot of energy and an upbeat attitude
• 4 pm - 8 pm Tuesday and Thursday / Saturday 12 pm - 5 pm
• 3:30 pm - 6:30 pm Saturday and Sunday
• Relief Staff 9 am - 3 pm with One Individual
• 12 pm - 4 pm Wednesday
• Monday - Friday 9:00am - 3:00pm
