Job summary

Enhabit Home Health & Hospice in Montana is seeking a skilled professional to expand referral networks and coordinate post-acute transitions. You will act as the primary liaison between Enhabit and physicians, hospitals, and senior care communities to ensure smooth patient journeys and high-quality outcomes.

The role emphasizes education of referral sources, compliance with Medicare guidelines, and building lasting partnerships while maintaining dependable transportation and in-office presence

Qualifications

• Bachelor's degree in healthcare administration, marketing, or related field is required.
• Graduate of an approved school of nursing, therapy, or social work is highly preferred.
• Minimum two years of healthcare business development, community relations, marketing, or sales experience.
• Understanding of Medicare home health and hospice regulatory guidelines.
• Valid driver's license and automobile liability insurance.

Responsibilities

• Serve as primary relationship manager with referral sources including physicians, hospitals, and health systems.
• Educate referral sources on Enhabit services and program requirements to support post-acute transitions.
• Grow referrals and improve patient outcomes and satisfaction.
• Maintain professional relationships with physicians, hospitals, and senior living communities.
• In-office attendance is essential to achieve objectives.

Responsibilities

Serves as the primary relationship manager between the company and referral sources including physicians, physician practices, hospitals, health systems, inpatient facilities, senior living communities, professional associations, and other healthcare organizations or progressional. Supports referral growth by providing awareness and education of company services and program requirements to referral sources, patients, families, caregivers and community partners to help navigate post-acute care transitions with the goal of improving patient outcomes, supporting care transition, and promoting exceptional quality and patient / referral source satisfaction.

Qualifications

• Required to have Bachelor's Degree in, healthcare administration, marketing, or related field or a graduate of an approved school of nursing, therapy or social work.
• Highly preferred graduate of an approved school of nursing, therapy or social work.
• Required minimum two years of experience in healthcare business development, community relations, marketing, or sales.
• Required demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing Medicare certified home health and hospice operations.
• Required to have and maintain a valid state driver license in the current state of residence.
• Required to have and maintain automobile liability insurance, as required by law.
• Required strong interpersonal, communication and presentation skills.
  • If Clinical:
  • Required to have and maintain clinical license, per state requirements to complete this job in accordance with applicable board's rules.
• Highly preferred experience in home health, hospice, post-acute care, or related healthcare services.
• Preferred previous experience using electronic referral systems and Homecare Homebase (HCHB).
• Preferred established relationships with physicians, hospitals or senior living communities and other referral sources.
• Must maintain dependable transportation in good working condition and be able to safely drive in all types of weather conditions.
• Oral communication, written communication, fluency in English, active listening.
• Demonstrated intermediate technology skills. Especially Microsoft office, including word and excel.
• Other than for designated full-time remote positions, in-office attendance is essential in order to achieve Enhabit objectives, including but not limited to, effective supervision, teamwork, task completion, and other position objectives.
