Job summary

Lotus-Like Psychotherapy & Wellness Center in Fresno is seeking a social worker/community liaison to support clients via case management, community engagement, and resource development. This hybrid role requires flexibility in hours and a passion for serving marginalized communities.

Qualifications include a Master’s degree in counseling or social work, strong documentation, and proficiency with EHR systems. Experience with CalFresh navigation and bilingual abilities are a plus.

Qualifications

• Master's degree in Counseling, Social Work, or related field is required.
• Experience engaging with community resources and clients.
• Bilingual in English/Spanish is a plus.

Responsibilities

• Case management, including coordination of client care and referrals across internal and external service providers.
• Assisting clients with navigating the process of signing up for public benefits such as CalFresh.
• Serving as a community liaison, building and maintaining relationships with community organizations, agencies, and resources.
• Supporting special projects as assigned by clinical leadership.
• Event coordination, including planning and logistics for community events.
• Creation of promotional materials for monthly groups and community events.
• Maintaining and regularly updating an internal resource directory of community services.
• Maintaining and coordinating the practice's client food pantry across local offices.
