Job summary

Tennessee Quality Care seeks a Community Liaison to build strong ties with hospitals, physicians, SNFs, assisted living facilities and other referral sources in Columbia, TN and surrounding areas. You’ll promote home health and hospice services, identify referral opportunities and educate partners to ensure smooth patient transitions.

You will collaborate with local executives to grow census, develop education programs and maintain professional relationships across the healthcare community,

Qualifications

  • Bachelor’s degree in Marketing, Business Administration, Communications or a related field.
  • At least two years of home care related sales or community education experience, in-home care experience is preferred.
  • Demonstrated ability to drive client census growth, develop business and professional relationships.
  • Experience in managing business prospecting software and identifying opportunity for account target selection and volume growth.
  • Ability to manage coordination of multiple lines of business.
  • Proven track record of exceeding established objectives and goals.
  • Excellent communication, interpersonal and salesmanship skills.
  • Self-confident, energetic, tenacious and resolute, with a high sense of urgency and a strong drive to produce results.
  • Ability to multi-task under tight deadlines.
  • Strong communication skills and interpersonal skills

Responsibilities

  • Promote Skilled Services provided under Home Health Benefits, such as, Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Home Health Aide and Medical Social Worker.
  • Demonstrate relationship development and public relation skills: research, cultivate, implement, produce and drive comprehensive, effective and coordinated community awareness with Acute Care discharge planners, Skilled Nursing Facilities, Senior living facilities, physicians and business professionals both in and outside of the healthcare community.
  • Collaborate with the Executive Directors and Directors of Clinical Services in their local markets to identify business development opportunities and coordinate education and partnership meetings
  • Develop and execute an effective and targeted plan that incorporates the entire local leadership team and generates increased census and promotes our positive image in the community as the leading homecare provider
  • Provide weekly reporting to the Health Partnership Director and Executive Directors with respect to activity and results for the week

The Community Liaison for Tennessee Quality Care serves as a vital link between the organization and the local healthcare community in the Columbia, TN territory. This role is responsible for building and maintaining strong relationships with hospitals, physicians, skilled nursing facilities, assisted living communities, and other referral sources to promote home health and/or hospice services.


The Community Liaison drives business growth by identifying new referral opportunities, educating healthcare partners on available services, and ensuring a seamless transition of care for patients. This individual represents Tennessee Quality Care with professionalism, integrity, and a deep understanding of patient-centered care.


Territory: Columbia, Tennessee (and surrounding service areas)

What You’ll Do
