Job summary

Community Options, Inc. in Edison, NJ seeks a Community Integration Counselor to assist individuals with learning daily living, social, emotional, and physical skills in the community. The role involves direct support, collaboration with case managers, and facilitating meaningful activities.

Qualifications

• High School Diploma or GED required; Bachelor’s degree preferred.
• Valid driver’s license with satisfactory driving record.
• Minimum 2 years of experience supporting individuals with intellectual/developmental disabilities.
• Excellent verbal and written communication skills.
• Ability to stay calm and exercise good judgment in crises.

Responsibilities

• Support individuals to increase independence and daily living skills.
• Meet waiver participants regularly in residence, community, or office settings.
• Collaborate with the Case Manager to develop social, emotional, physical, and personal programs.
• Provide ongoing support for community integration efforts.
• Document progress toward community integration and outcomes.
• Build a support network between the individual and the community.
