Job summary

Community Options, Inc. in Lawrenceville, NJ, seeks a Community Integration Counselor to help individuals develop life skills across physical, intellectual, emotional, and social domains. The role delivers individualized services designed to help participants manage challenges while living in the community.

Starting pay is $18.00 per hour with an 8am–4pm schedule. Responsibilities include supporting independence, meeting waiver participants, coordinating with the Case Manager, documenting

Qualifications

• High School Diploma or GED required.
• Bachelor’s Degree preferred.
• Valid driver’s license with satisfactory driving record.
• Minimum of 2 years of experience supporting individuals with intellectual and developmental disabilities.
• Excellent verbal and written communication skills.

Responsibilities

• Support individuals to increase independence.
• Meet with waiver participants in residences, communities, or offices.
• Collaborate with Case Manager to design programs meeting social, emotional, physical, and personal needs.
• Provide ongoing support for community integration efforts.
• Document progress toward community integration.
• Build a support network between individuals and the community.
• Ensure physician forms are completed for medical appointments.
• Administer medications and maintain documentation.
• Coordinate leisure and volunteer opportunities.
• Monitor health and medical needs and report concerns.
• Transport individuals to medical appointments and activities.
• Ensure program documentation and billable records are completed accurately.
• Cooperate with licensee and staff during inspections or investigations.
• Additional tasks may be assigned.
