Job summary

Community Options, Inc. in Pittsburgh, PA is seeking a Community Integration Counselor to assist individuals in developing life skills and independence within community settings.

The role focuses on individualized supports, documentation, and coordination of services to promote integration. The position requires a high school diploma or GED, a valid driver’s license, and at least two years of relevant experience with individuals with intellectual and developmental disabilities.

Qualifications

• High school diploma or GED; bachelor's degree preferred.
• Valid driver's license with a satisfactory driving record.
• Minimum of two years of relevant experience supporting individuals with intellectual and developmental disabilities.
• Ability to establish a comfortable and supportive relationship with individuals receiving supports.
• Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing.

Responsibilities

• Support and encourage individuals to increase independence.
• Meet with waiver participants regularly in residence, the community, or an office setting.
• Provide ongoing support for community integration efforts.
• Document progress toward community integration.
• Coordinate leisure and volunteer opportunities.
• Drive individuals to medical appointments and activities in the community.
• Ensure program documentation and billable records are completed timely.
• Cooperate with license and department staff during inspections.
