Job summary

Indiana Alliance of YMCAs is seeking a Community Integrated Health Manager to lead daily delivery of evidence-based health programs under the Rural Health Transformation Program (RHTP) grant. You will work with YMCAs and community sites to implement chronic disease prevention priorities and align with statewide strategy.

The role requires coordinating across multiple sites, ensuring data accuracy and HIPAA compliance, and frequent Indiana travel.

Qualifications

• BA/BS in Public Health, Community Health, Health Promotion, Exercise Science, or related field required.
• Minimum of 3 years' experience delivering community health programs, ideally chronic disease prevention.
• Experience coordinating delivery across multiple sites, preferably rural communities.
• Strong facilitation, coaching, and relationship-building skills.
• Proficient in Microsoft Office and familiar with data tracking systems.
• Ability to travel regularly across Indiana, including evenings/weekends.
• CPR and First Aid certification required; YMCA training completed within deadlines.

Responsibilities

• Direct in-person and virtual delivery of evidence-based chronic disease prevention programs across YMCAs and community sites.
• Advance prevention activities in partnership with local YMCA staff and sites.
• Prioritize SNAP recipients and SNAP-eligible communities in planning and delivery.
• Provide technical assistance, training, and coaching to local Y staff delivering interventions.
• Monitor program fidelity and engagement; elevate implementation barriers to the State Director.
• Support wearable device distribution and use at program sites in collaboration with the Data/Wearables Manager.
• Track and submit program data, attendance, and outcomes for grant reporting.
• Maintain accurate program records; ensure HIPAA compliance and YMCA guidelines.
• Perform other duties as needed for the Indiana Alliance of YMCAs.

SUMMARY

The Community Integrated Health Manager is a grant-funded position responsible for leading the day-to-day delivery of evidence-based, community-integrated health programming under the Rural Health Transformation Program (RHTP) grant. This position works directly with local YMCAs and community-based sites to implement a suite of evidence based chronic disease programs and supports site-level execution of the grant's chronic disease prevention priorities. The Manager reports to the State Director of Rural Health and serves as the primary link between statewide strategy and on-the-ground program delivery

RESPONSIBILITIES, ESSENTIAL FUNCTIONS & KEY DELIVERABLES

Program Delivery & Implementation

• Direct in-person and virtual delivery of evidence-based, chronic disease prevention programs across the lifespan at YMCAs and community-based sites.
• Advance evidence-based chronic disease prevention and community-integrated health activities in partnership with local Y staff and community sites.
• Prioritize SNAP recipients and SNAP-eligible communities in program planning and delivery.

Site & Staff Support

• Provide technical assistance, training, and coaching to local Y program staff and instructors delivering grant-funded interventions.
• Monitor program fidelity and participant engagement at delivery sites; elevate implementation barriers to the State Director of Rural Health.
• Support wearable device distribution and use at the program-site level in partnership with the Data/Wearables Manager.

Reporting & Compliance

• Track and submit program-level data, attendance, and outcomes to support monthly, quarterly, and annual grant reporting.
• Maintain accurate program records consistent with grant and YMCA documentation standards.
• Ensure local Y guidelines, including HIPAA compliance are implemented.
• Perform other duties/assignments as needed at the Indiana Alliance of YMCAs.

• BA/BS in Public Health, Community Health, Health Promotion, Exercise Science, or a related field required.
• Minimum of 3 years' experience delivering community health programs, ideally including evidence-based chronic disease prevention or cardiometabolic health interventions (e.g., Diabetes Prevention Program, EnhanceFitness, blood pressure self-monitoring).
• Experience coordinating program delivery across multiple sites, preferably in rural communities.
• Strong facilitation, coaching, and relationship-building skills with program staff and community partners.
• Proficient in Microsoft Office Suite and comfortable using program tracking or data collection systems.
• Ability to travel regularly across assigned Indiana region, including occasional evenings/weekends for programming.
• Must have or be able to obtain CPR and First Aid certification.
• Completion of all position-specific and mandatory YMCA training within specified deadlines.
