Job summary

PROVAIL in Washington state is seeking a Community Inclusion Coordinator to support individualized skill development, independent living, and community integration for clients. This role emphasizes person-centered planning and helping individuals engage in local activities.

This full-time position requires a HS Diploma/GED, a valid Washington driver's license with a good driving record, and the ability to pass a background check.

Qualifications

• Must have HS Diploma/GED and at least 1 year of work experience.
• Must possess a valid WA driver\'s license with good driving record and access to a vehicle.
• Must be able to pass a criminal history background check.

Responsibilities

• Person-Centered Planning and Documentation: develop and maintain individualized community inclusion plans; complete and submit required documentation; ensure services align with client plans.
• Community Inclusion and Skill Development: provide services to individuals or small groups; help clients identify, access, and engage in activities promoting socialization and independence.
• Community Engagement and Collaboration: build relationships with clients and partners; travel between sites and attend off-site meetings as needed.
• Accessibility, Equity & Inclusion: contribute to an inclusive workplace; collaborate across departments; participate in equity and inclusion learning.
• Additional Responsibilities: perform related duties as assigned.
