Qualifications

• Demonstrates empathy and respectful interactions with individuals served.
• Clear written and verbal communication with stakeholders.
• Ability to implement Behavior Support Plans (BSP) with clinical support.

Responsibilities

• Provide support services to individuals in community-based program.
• Assist with activities of daily living and personal care.
• Implement crisis prevention and intervention strategies as approved.
• Develop and evaluate Individual Plans (IP) with the team.
• Complete required paperwork accurately and on time.
• Operate program safety protocols and maintain facility upkeep.
• Provide transportation for individuals as needed.
• Participate in training and quality improvement initiatives.
