Responsibilities

• Applies age- and population-specific competencies.
• Interacts professionally in written and oral format with stakeholders.
• Provides a therapeutic environment with dignity and respect.
• Assists individuals with activities of daily living.
• Manages behavioral crises with BSP and crisis-intervention strategies.
• Works with multiple individuals to encourage participation and support.
• Develops, implements and evaluates Individual Plans (IP) and provides case management.
• Completes required paperwork in a timely manner and follows managerial direction.
• Implements program safety protocols.
• Maintains on-call responsibilities and acts as interim Program Manager when needed.
• Provides transportation to individuals served and assists with vehicle maintenance.
• Completes trainings and certifications as required.
• Participates in quality improvement to increase productivity and service quality.
• Maintains program facilities and submits maintenance requests.
• Adheres to Journey Found policies and protects consumer health information.
