Job summary

The Omaha Tribe of Nebraska is seeking a Youth Health & School-Based Support Community Health Worker to engage tribal youth, partner with schools, youth programs, and community organizations, and promote healthy behaviors through culturally grounded approaches.

You will educate on substance use prevention, sexual health, and risk reduction, support youth wellness activities, and help connect at-risk youth to services while maintaining confidentiality and documenting outreach activities.

Qualifications

• Must have a High School Diploma or GED.
• Must complete 75-hour State approved Nursing Assistant course and be on State Registry.
• Must complete 40-hour Medication Aide course and be on State Registry.
• Must possess a valid driver’s license and be insurable.
• Must pass background check and pre-employment drug screen.

Responsibilities

Job description

SUMMARY: The Youth Health & School-Based Support Community Health Worker (CHW) provides outreach, education, and engagement services to support the health, safety, and well-being of tribal youth. This role partners with schools, youth programs, and community organizations to promote healthy behaviors, reduce risk factors, and encourage positive youth development through culturally grounded approaches.

ESSENTIAL DUTIES and/or RESPONSIBILITIES include the following:

• Work in collaboration with tribal youth programs and schools to engage youth in positive, healthy, and culturally relevant activities.
• Provide education to youth on topics including substance use prevention (drugs and alcohol), sexual health and prevention of unprotected sex, motor vehicle safety, suicide prevention, and overall risk reduction.
• Support the development and facilitation of youth wellness activities, prevention programs, and outreach events.
• Build trusting relationships with youth to encourage open communication, resilience, and healthy decision-making.
• Promote mental, emotional, physical, and spiritual wellness using culturally appropriate and trauma-informed approaches.
• Identify youth who may be at risk and connect them to appropriate support services and resources.
• Collaborate with schools, families, and tribal programs to support coordinated youth wellness efforts.
• Assist with planning and participating in community health fairs, school events, and prevention campaigns.
• Maintain accurate documentation of outreach activities, education sessions, and referrals while maintaining confidentiality.
• Other duties as assigned.

EDUCATION and/or EXPERIENCE:

• Must have a High School Diploma or G.E.D.

• 75-hour State approved Nursing Assistant course and placed on State Registry. 40 hours Medication Aide course and placed on State Registry.

CERTIFICATES, LICENSES, and/or REGISTRATIONS:

• Must be a Certified Nursing Assistant and a Medication Aide certificate in the State of Nebraska
• Possess or pass test for CPR. Optional A.C.L.S.
• Must possess a valid driver's license and be insurable.
• Must pass background check and pre-employment drug screen.

PHYSICAL REQUIREMENTS:

• Must have the ability to lift, push a Wheelchair in and out of a vehicle.
• Must comply with the State of Nebraska Community Health Worker Grant requirements.

KNOWLEDGE, SKILLS, and/or ABILITIES:

• Experience working with youth in school, community, or recreational settings preferred.
• Strong communication, mentorship, and relationship-building skills.
• Ability to engage youth in a respectful, culturally responsive, and nonjudgmental manner.
• Knowledge of youth risk behaviors and prevention strategies preferred.
