Job summary

The Omaha Tribe seeks a Maternal & Child Health Community Health Worker (CHW) to provide culturally responsive support to pregnant individuals, postpartum parents, infants, and families. • Must have a High School Diploma or GED.
• Complete 75-hour Nursing Assistant course and 40-hour Medication Aide course.
• CHW certification preferred or willing to obtain within stated timeframe.
• Experience in public health or tribal health is preferred.

Responsibilities

• Assist clients with coordinating and accessing transportation to prenatal, postpartum, and pediatric appointments.
• Provide culturally appropriate educational materials related to pregnancy, postpartum care, infant health, and child development.
• Offer ongoing emotional support, encouragement, and advocacy for clients during pregnancy and early childhood.
• Connect individuals and families to community resources including healthcare, nutrition programs, housing assistance, and social services.
• Support care coordination by communicating with healthcare providers and service organizations as appropriate.
• Conduct outreach to promote maternal and child health services within the community.
• Uphold confidentiality and respect for cultural values, traditions, and practices.
• Promote early prenatal care, postpartum follow-up, breastfeeding, safe sleep, and infant wellness.
• Document encounters in the electronic health record and assist with grant/program reporting.
• Participate in outreach events, home visits, quality improvement activities, and multidisciplinary team meetings.
• Other duties as assigned.

SUMMARY:

The Maternal & Child Health Community Health Worker (CHW) provides culturally responsive support to pregnant individuals, postpartum parents, infants, and families within the community. This role focuses on improving maternal and child health outcomes by assisting with access to care, providing health education, and connecting families to essential resources and services.

ESSENTIAL DUTIES and/or RESPONSIBILITIES include the following:

• Assist clients with coordinating and accessing transportation to prenatal, postpartum, and pediatric appointments.
• Provide culturally appropriate educational materials related to pregnancy, postpartum care, infant health, and child development.
• Offer ongoing emotional support, encouragement, and advocacy for clients throughout pregnancy and early childhood.
• Connect individuals and families to community resources, including healthcare, nutrition programs, housing assistance, and social services.
• Support care coordination by communicating with healthcare providers and service organizations as appropriate.
• Conduct outreach and engagement activities to promote maternal and child health services within the community.
• Uphold confidentiality and respect for cultural values, traditions, and practices.
• Promote early prenatal care, postpartum follow-up, breastfeeding, safe sleep, and infant wellness.
• Document encounters accurately in the electronic health record and complete grant/program reporting.
• Participate in outreach events, home visits, quality improvement activities, and multidisciplinary team meetings.

QUALIFICIATION REQUIREMENTS:

EDUCATION and/or EXPERIENCE:

• Must have a High School Diploma or G.E.D.
• 75-hour State approved Nursing Assistant course and placed on State Registry. 40-hour Medication Aide course and placed on State Registry.
• Community Health Worker certification preferred or willingness to obtain.
• Experience in public health, maternal-child health, community outreach, or tribal health preferred.
• Strong communication, organization, and computer skills.

CERTIFICATES, LICENSES, and/or REGISTRATIONS:

• Must be a Certified Nursing Assistant and a Medication Aide certificate in the State of Nebraska
• Possess or pass test for CPR. Optional A.C.L.S.
• Must possess a valid driver's license and be insurable.

PHYSICAL REQUIREMENTS:

• Frequent sitting, standing, walking, bending, and lifting to 30 pounds.
• Must comply with the State of Nebraska Community Health Worker Grant requirements.
• Must have the ability to lift, push a wheelchair in and out of a vehicle.

KNOWLEDGE, SKILLS, and/or ABILITIES:

• Maintain required documentation within organizational timelines.
• Meeting maternal and child health outreach goals.
• Maintain client confidentiality and comply with HIPAA and Tribal policies.
• Complete required continuing education and CHW training.
