Aspire Indiana Health, Inc. is seeking a Community Health Worker – Transportation in Carmel, Indiana. You will provide and coordinate transportation for clients and staff, promoting a medical home model and connecting people to Aspire services.

Requires a High School Diploma, driving experience, and ability to drive in varied weather; 15-passenger van/bus background preferred. We offer comprehensive benefits and opportunities to impact community health.

Formación

• High School Diploma or equivalent required; Bachelor’s Degree preferred in social work, psychology, counseling or related field.
• One year of related driving experience required.
• Must be able to drive in varying weather conditions.

Responsabilidades

• Coordinate transportation services for clients and employees.
• Coordinate Aspire fleet maintenance and ensure safe driving operations.
• Promote a medical home model with healthcare team involvement.
