Job summary

The CHW Supervisor at Montefiore Einstein CHW Institute will oversee the daily management of the CHW workforce, guiding performance, recruitment, orientation and coaching across multiple clinical sites. This role partners with staff and senior management to develop, maintain and grow the CHW program.

Travel to sites is required; bilingual candidates (Spanish or French) are preferred and will help address diverse community needs.

Qualifications

• Associates Degree required; Bachelor's preferred.
• 2-3 years supervisory experience or 3 years direct CHW/care coordination experience.
• Proficiency with Microsoft Word, PowerPoint and Excel.
• Experience in the public health field; familiarity with community health workers preferred.
• Knowledge of the local community, culture and resources, is favorable.
• Bilingual in Spanish or French preferred.

Responsibilities

• Oversee recruitment, orientation and training of CHW staff.
• Ensure day-to-day operations align with CHW Center goals, protocols and policies.
• Assign, manage and monitor caseloads or CHW staff.
• Coordinate and participate in case reviews, trainings and implementation planning.
• Develop, operationalize and optimize strategies to address social support services.
• Ensure timely and accurate CHW staff documentation and reporting; monitor program outcomes.
• Conduct regular meetings with CHW Team and stakeholders for feedback and navigation of challenges.
• Effectively communicate implementation successes and challenges with the Program Manager.
• Provide performance feedback and conduct annual performance reviews for CHW staff.
